Voluntary incident reporting is a source of learning, but the use of clinical triggers is a more effective method for measuring adverse events. We continue to automate clinical triggers with Risk Trigger (compatible with the IHI Global Trigger Tool*). Risk Trigger enables healthcare system partners to drive down harm and costs by focusing on areas such as glycemic management, anti-coagulant management, perinatal management, and medication management.
