Case Studies
In 2009, Adventist Health System began using Risk Trigger to gauge the number, percentages, types, and levels of severity of adverse events occurring in its facilities. Reviewing 11,392 patient records between January 2009 and December 2010, Adventist Health System found 4,418 adverse events, most of which were associated with medication-related glycemic management. They formed the Glycemic Collaborative in 2010 to address the problem. Read more
Since May 2008, Maine Medical Center has used a variety of cultural measurement and intervention tools to improve their patient safety culture. Maine Medical Center has partnered with Pascal Metrics to administer more than 1,000 surveys, complete more than 350 cultural observations, and train more than 1,200 employees in teamwork skills. Coupled with targeted interventions in three units, data suggest improvements in patient safety and hospital culture. Read more
In 2009, the Providence St. Peter Hospital surgical acute care unit in Olympia, Washington, became the first site for the Accelerating Clinical Transformation Patient Safety Pilot Study. PSPH partnered with Pascal to administer the Safety Attitudes Questionnaire (SAQ) and to support teamwork training and improvements. Teamwork training led to significant improvements in patient and employee satisfaction, perceptions of safety culture, and clinical outcomes. Read more
The Rhode Island ICCollaborative partnered with Pascal Metrics to administer the SAQ to 23 units in the fall of 2007. Units were encouraged to develop action plans based on insights from the safety culture debriefing tool provided on HealthBench. In 2008, the Collaborative resurveyed all 23 units and found that the 9 that debriefed and developed action plans experienced significant improvements in both SAQ domain scores and clinical outcomes such as CLA-BSI rates and VAP rates.
